Job summary

PharMerica in Indianapolis is seeking a PRN Pharmacist to join a closed-door pharmacy team serving long-term care and senior living clients. You will evaluate orders, provide clinical recommendations, and maintain patient profiles in a non-retail setting.

Qualifications

• Bachelor’s degree in Pharmacy; PharmD preferred.
• One year experience in long term care desired.
• Current pharmacist license in the state of practice.
• Knowledge of CMS, DEA, FDA guidelines and regulatory standards.
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and routes.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date resident profiles including medical history and current medications.
• Ensure laboratory tests for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and healthcare staff for clinical support.
• Document all communications and interventions in the medical record.
• Implement and enforce policies for safe handling and dispensing of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to the Pharmacy Manager.
• Educate and support clinical staff on safe medication procedures.
• Perform other pharmacy-related tasks as assigned.
