Job summary

PharMerica in Milwaukee, WI is seeking a full-time Pharmacist to join our closed-door pharmacy team serving long-term care and senior living clients. You will review orders, provide clinical recommendations, and maintain patient profiles to ensure safe and effective therapies.

Qualifications

• Bachelor’s degree in Pharmacy (PharmD preferred).
• One year experience in long term care desired.
• Current pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement, and effective written and verbal communication.
• Position will be posted for a minimum of 7 business days.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff regarding medication therapy changes.
• Maintain up-to-date patient/resident profiles with medical history, labs, and current meds.
• Ensure labs for monitoring high-risk meds are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical support.
• Document all communications and interventions in the medical record, including family interactions.
• Enforce policies for safe handling, dispensing, and clinical oversight of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse events to Pharmacy Manager.
• Educate and support clinical team on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
