Job summary

PharMerica is seeking a Part-Time Pharmacist in Grand Prairie, TX to join a closed-door pharmacy team serving long-term care and senior living clients. You will evaluate orders, provide clinical recommendations, and maintain patient profiles in a non-retail setting.

You will collaborate with physicians and nurses to optimize therapy, document all interventions, and ensure compliant, safe medication management across the facility. Preference given to candidates with LTC experience and TX license.

Qualifications

• Licensed pharmacist in the state of practice (TX).
• Bachelor's in Pharmacy or PharmD preferred.
• One year of long-term care pharmacy experience desired.

Responsibilities

• Evaluate physician medication orders for drug appropriateness and dosing.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date patient/resident profiles and medical history.
• Ensure lab tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and staff for medication therapy optimization.
• Document communications and interventions in patient records.
• Implement policies for safe handling, dispensing, and management of medications.
• Ensure compliance with state/federal pharmacy regulations.
