PharMerica in Valdosta, GA is seeking a PRN Pharmacist to join a closed-door, non-retail team serving long-term care and senior living clients. The position is on‑call, providing clinical and dispensing support in a collaborative environment.

You will evaluate orders, optimize therapy, maintain patient profiles, and ensure compliance with state and federal regulations. Ideal candidates hold a PharmD or Bachelor in Pharmacy with active Georgia licensure and LTC experience; on-site teamwork, and

Qualifications

• Bachelor's Degree in Pharmacy is required; PharmD preferred.
• One year experience in long-term care desired.
• Current and valid pharmacist license in state's practice.
• Knowledge of CMS/DEA/FDA guidelines.
• Proficiency with EMR and pharmacy systems.
• Strong clinical judgement and communication.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, and interactions.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain resident profiles with medical history, labs, and meds.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians and nurses to support care.
• Document communications and interventions in the medical record.
• Enforce safe handling, dispensing, and medication management policies.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to the Pharmacy Manager.
• Educate clinical staff on safe medication procedures.
• Perform other pharmacy-related tasks as assigned.
