PharMerica in Washington, NC is hiring a full-time Pharmacist to join our team in a non-retail, long-term care focused setting. You will review orders, ensure safety, collaborate with physicians and facility staff to optimize therapy.

Formación

• Bachelor's Degree in Pharmacy required; Pharm D preferred.
• One year of long-term care experience desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS, DEA, and FDA guidelines.
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgment and communication skills.

Responsabilidades

• Evaluate physician medication orders for appropriateness, dosage, interactions, and route.
• Provide clinical recommendations to physicians and facility staff regarding therapy changes.
• Maintain up-to-date patient/resident profiles with medical history, labs, and meds.
• Ensure lab tests for high-risk meds are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and other professionals for clinical support.
• Document communications and interventions in the medical record.
• Enforce policies for safe procurement, storage, and disposal of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events as required.
• Educate facility staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
