Qualifications

• Bachelor’s Degree in Pharmacy; PharmD preferred.
• One year experience in long-term care desired.
• Current pharmacist license in state of practice.
• Knowledge of drug therapy guidelines and safety standards.
• Proficiency with electronic medical records and pharmacy systems.
• Strong clinical judgement and communication skills.

Responsibilities

• Evaluate physician orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff on medication therapy changes.
• Maintain up-to-date patient profiles including medical history and current meds.
• Ensure labs for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and team for clinical support.
• Document communications and interventions in the patient record.
