Qualifications

• Bachelor’s Degree in Pharmacy is required.
• Current and valid pharmacist license in the state of practice.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes.
• Maintain up-to-date patient profiles, including medical history, labs, current meds, and OTC/home remedies.
• Ensure required lab tests for monitoring high-risk meds are ordered and communicated.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical support.
• Document all communications and interventions in the patient’s medical record.
