Job summary

PharMerica in Horsham, PA is hiring a full-time Pharmacist to join our closed-door, long-term care team. You will review orders, provide clinical recommendations, and collaborate with physicians to optimize therapy. Schedule: Mon-Fri 3:00pm-11:00pm with every other weekend.

In addition to a $2.50/hr shift differential, we offer Medical/Dental/Vision, HSA/FSA, tuition reimbursement, 401(k), PTO, DailyPay, pet insurance, and wellness programs. PA license required; PharmD preferred.

Qualifications

• Bachelor’s Degree in Pharmacy or PharmD preferred.
• Current and valid pharmacist license in the practice state.
• Knowledge of drug therapy guidelines and regulatory standards.
• Experience in long-term care is desirable.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage and interactions.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain patient/resident profiles with medical history and current medications.
• Ensure lab tests for high-risk meds are ordered and communicated to the team.
• Collaborate with physicians, nurses, and other professionals.
• Document communications and interventions in the medical record.
• Implement policies for safe handling, dispensing and management of medications.
• Ensure compliance with state and federal regulatory requirements.
• Report medication errors to Pharmacy Manager, educate the team.
