Job summary

PharMerica in Leesport, PA, is hiring a full-time Pharmacist for a non-retail, long-term care setting. You will evaluate orders, advise on therapy changes, maintain resident profiles, ensure monitoring labs, collaborate with care teams, and educate staff on safe medication handling.

Qualifications

• Bachelor’s Degree in Pharmacy; PharmD preferred.
• One year experience in long-term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines and regulatory standards.
• Proficiency in electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and route.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date patient/resident profiles and med history.
• Ensure lab tests for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and other professionals for clinical support.
• Document communications and interventions in medical records.
