Job summary
HCA Florida Fawcett Hospital seeks a skilled Clinical Pharmacist to provide comprehensive clinical pharmacy services for patients and oversee safe procurement, preparation, labelling, distribution and control of medications. You will collaborate with the healthcare team to optimize therapy and outcomes.
The role requires a Pharm D or B.S. in Pharmacy, licensure in Florida, and experience in acute-care settings.
Qualifications
- Bachelor of Science or Pharm D degree in Pharmacy is required or preferred.
- Experience in acute-care hospital setting or residency preferred.
- Current FL Pharmacist license or license eligible.
Responsibilities
- Coordinate and assist with development, implementation, and maintenance of clinical pharmacy programs to meet patient population needs.
- Provide clinical pharmacy support and execute programs for all patient areas.
- Review medication orders and verify drug appropriateness, dosage, route and interactions; adjust as needed.
- Ensure safe dispensing of medications according to patient age.
- Monitor narcotics transactions and reconcile quantities of dispensed narcotics.
- Assist with physician consults and retrieve reliable drug information as needed.
- Communicate effectively with physicians and the interprofessional team to meet patient needs.
