Job summary

Mercy Hospital Jefferson in Missouri seeks a Clinical Pharmacist to join our hospital team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, patient-centered medication therapy and serve as a resource for drug information.

You’ll participate in Medication Therapy Services, education initiatives, and quality improvement projects while adhering to Mercy's mission, standards, and regulatory requirements.

Qualifications

• PharmD or BS in Pharmacy from an accredited school.
• Current pharmacist license in the applicable state.
• Two or more years of clinical pharmacy experience (preferred).
• Completion of a PGY-1 Pharmacy Residency (preferred).

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Collaborate with prescribers, nurses, and patients on dosing and therapy.
• Develop medication plans to minimize risk and optimize outcomes.
• Provide drug information and act as a resource for care teams.
• Participate in Medication Therapy Services and quality improvement.
