Qualifications

• Required: BS or PharmD from an accredited school.
• Current pharmacist license in the applicable state.
• Two+ years clinical pharmacy experience preferred; PGY-1 residency preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Collaborate with clinicians to optimize treatment and dosing.
• Provide medication information and safety guidance to care teams.
• Precept and mentor pharmacy students and residents as needed.
• Participate in quality improvement, policy reviews and MUEs.
