Job summary

South Texas Health System seeks a Clinical Pharmacist to join the multidisciplinary team, providing antimicrobial stewardship support, medication safety, and dosing optimization. The role includes assisting with medication histories, monitoring therapies, and drug information for patients.

Responsibilities include responding to Codes, filling orders, supervising staff, and potentially assuming Director of Pharmacy duties when needed.

Qualifications

• Completion of a general 1-year residency, specialty residency, and/or fellowship or at least 3 years of hospital pharmacy experience
• Ability to work independently and handle stress appropriately.
• Ability to communicate effectively (verbal and written), preferably in both English and Spanish.
• Knowledge of laws, rules and regulations that govern the practice of pharmacy
• Commitment to Compliance Program and Code of Conduct and reporting requirements

Responsibilities

• Serve as resource for antimicrobial stewardship, medication safety, and optimized dosing.
• Assist with medication histories and monitor patient therapies.
• Respond to Codes (Code Blue/Sepsis/Stroke/STEMI) and prepare needed medications.
• Direct supervising staff and, in absence of Director of Pharmacy, assume essential responsibilities.
