Job summary

Mercy is seeking a Clinical Pharmacist I – Float (Full-Time) to join an interdisciplinary health care team. You will review patient records, optimize pharmacotherapy, and provide MTM services to ensure safe, effective medication therapy across care settings.

This role emphasizes collaboration with physicians and nurses, compliance with standards, and ongoing professional development. No weekends or holidays with a $10,000 sign-on bonus paid at milestones.

Qualifications

• PharmD or BS in Pharmacy from an accredited school.
• Active pharmacist license in the state of practice.
• 1–4 years of clinical pharmacy experience preferred.

Responsibilities

• Review patient records to evaluate the appropriateness and effectiveness of medication therapy.
• Assess patient conditions to ensure comprehensive treatment of all clinical concerns.
• Develop and optimize pharmacotherapy plans to improve outcomes and minimize adverse effects.
• Monitor drug therapy through chart review and interdisciplinary collaboration; intervene when appropriate.
• Consult with providers, nurses, and patients on medication selection, dosing, administration, interactions, and side effects.
• Evaluate treatment outcomes and adjust therapy recommendations as needed.
• Maintain accurate and timely documentation of patient care activities.
• Provide Medication Therapy Management (MTM) services, including dose, route, schedule, contraindication, and interaction assessment.
