Mercy Hospital Springfield is seeking a clinically driven Pharmacist to join our hospital-based team on an evening block schedule. You will collaborate with physicians, nurses, and other clinicians to optimize medication therapies and improve patient outcomes within a top-tier health system.

The role emphasizes practicing at the top of your license, patient-focused care, and engagement in formulary management, education for patients and staff, and participation in quality initiatives.

Formación

• BS in Pharmacy or PharmD from an accredited pharmacy program.
• Current pharmacist license in the state of practice (or eligibility).
• Preferred: 2+ years of clinical pharmacy experience; PGY-1 residency; Missouri MT Services Certificate within 90 days for MO pharmacists.

Responsabilidades

• Review medication therapies for safety, effectiveness, and appropriateness.
• Collaborate with physicians, nurses, and clinicians to optimize care.
• Identify and resolve medication-related issues and interactions.
• Monitor patient responses and recommend therapy adjustments.
• Provide medication education to patients and providers.
• Serve as a clinical resource on formulary management and evidence-based practices.
• Participate in quality improvement and patient safety programs.
• Precept students and residents; support education activities.
• Maintain accurate clinical documentation and use Epic, Micromedex, and pharmacy automation.
