Mercy Hospital Springfield is seeking a Clinical Pharmacist to join our award-winning pharmacy team on an evening block schedule. You will collaborate with physicians, nurses, and other clinicians to optimize medication therapies and improve patient outcomes in a fast-paced hospital setting.

You'll practice at the top of your license, contribute to formulary management and education, and participate in quality improvement, patient safety programs, and learner precepting within a supportive,

Competenze

• BS in Pharmacy or PharmD from an accredited school of pharmacy.
• Current pharmacist license in the state of practice (or eligibility for licensure).
• Two or more years of clinical pharmacy experience.
• PGY-1 Residency training.

Mansioni

• Reviewing and evaluating medication therapies for safety, effectiveness, and appropriateness
• Collaborating with physicians, nurses, and other clinicians to optimize patient care
• Identifying and resolving medication-related issues, adverse reactions, and drug interactions
• Monitoring patient responses to therapy and recommending adjustments when appropriate
• Providing medication education and consultation to patients and healthcare providers
• Serving as a key clinical resource on formulary management, medication utilization, and evidence-based practices
• Participating in quality improvement initiatives, medication-use evaluations, and patient safety programs
• Supporting pharmacy learners through student and resident education opportunities
• Maintaining accurate clinical documentation and utilizing technology systems including Epic, Micromedex, and pharmacy automation platforms
