Job summary

Mercy is seeking a Clinical Pharmacist I – Float to join an interdisciplinary health care team and provide safe, evidence-based medication therapy. The role supports physicians, nurses, and pharmacy staff while upholding Mercy Service Standards.

No weekends or holidays and a $10,000 sign-on bonus (disbursed over 24 months) accompany a full-time, benefits-rich position in Wisconsin. Pharmacists enjoy teaching moments and professional growth.

Qualifications

• Education: PharmD or BS in Pharmacy.
• Active pharmacist license in the state of practice.
• 1–4 years of clinical pharmacy experience preferred.

Responsibilities

• Review patient records to evaluate medication therapy.
• Assess patient conditions to address clinical concerns.
• Develop and optimize pharmacotherapy plans to improve outcomes.
• Monitor drug therapy with chart reviews and team collaboration.
• Consult with providers, nurses, and patients on dosing and interactions.
• Evaluate treatment outcomes and adjust recommendations.
• Maintain accurate documentation of patient care.
• Provide MTM services including dose and contraindication assessment.
