Job description

LOCATION: Remote - This is a home-based, virtual position operating within the hours of 8:30am-5:00pm EST. Vaya Health welcomes applications from NC, SC, GA, TN, VA, MD, and FL.

GENERAL STATEMENT OF JOB

The Clinical Pharmacist supports high-quality, cost-effective healthcare for Medicaid, uninsured, and underinsured consumers in the Vaya region. Supervised by the Vice President of Pharmacy Operations, this role collaborates with medical team members including other pharmacists, care management staff, and nursing. Services include clinical chart review, interdisciplinary team participation, education, consultation, and involvement in Care Management and Quality Improvement projects. The Pharmacist also precepts pharmacy students.

ESSENTIAL JOB FUNCTIONS

  1. Pharmacy Consultations:
  • Review patient charts and medication lists in the Vaya care management platform.
  • Analyze pharmacy claims data and incorporate findings into consultations.
  • Communicate recommendations to relevant parties.
  • Support medication reconciliation efforts with nursing and care management staff.
  • Coordinate with pharmacies and prescribers as appropriate.
  • Other Clinical Roles:
    • Participate in weekly interdisciplinary care manager huddles.
    • Represent the pharmacy department on committees like the Clinical Strategy Committee.
    • Assist with drug information evaluation and review criteria adjustments.
    • Consult with nursing staff on pharmacologic best practices.
    • Develop new pharmacy programs supporting Vaya's Medicaid Tailored Plan.
    • Perform additional clinical duties as assigned.
  • Teaching and Training:
    • Collaborate on pharmacy training for regional staff and providers.
    • Update staff on pharmacotherapy best practices.
    • Supervise pharmacy student rotations.
    • Contribute to competency development for nursing and care staff.
  • Quality Improvement:
    • Make recommendations based on consultation data for quality projects.
    • Participate in quality activities and support accreditation efforts.

    KNOWLEDGE, SKILLS, & ABILITIES

    • Expertise in clinical pharmacy practice, mental health, substance use, developmental disabilities, and physical health disorders.
    • Ability to work independently in a remote setting.
    • Cultural sensitivity to I/DD, TBI, and behavioral health populations.
    • Understanding of healthcare disparities.
    • Problem-solving skills and adaptability.
    • Clear communication and professionalism.
    • Strong planning, organization, and interpersonal skills.
    • Public speaking confidence.
    • Proficiency with Microsoft Office and electronic health records.

    EDUCATION & EXPERIENCE REQUIREMENTS

    Doctor of Pharmacy (PharmD) from an accredited college, with:

    • Completion of a psychiatric clinical pharmacy residency, OR
    • PGY1 residency with at least 4 years of clinical practice focused on behavioral health or equivalent experience.

    Licensure/Certification:

    • Unrestricted pharmacist license from the North Carolina Board of Pharmacy.

    PHYSICAL REQUIREMENTS

    • Visual acuity for document review and computer work.
    • Physical activities include crouching, reaching, walking, talking, hearing, and hand movements.
    • Sedentary work with lifting up to 10 pounds, extended sitting.
    • Requires mental concentration.

    RESIDENCY REQUIREMENT: Must reside in NC, SC, GA, TN, VA, MD, or FL.

    SALARY: Commensurate with experience. Position is exempt, no overtime.
