Job summary

Mercy Hospital Jefferson in Wisconsin is seeking a Clinical Pharmacist to join the health care team. You will collaborate with physicians, nurses, and other professionals to ensure safe, effective, and patient-centered medication therapy.

In this PRN role, you will provide drug information, participate in Medication Therapy Management and education efforts, and support clinical initiatives while upholding Mercy's mission and service standards.

Qualifications

• Doctor of Pharmacy or PharmD; required licensure in applicable state.
• Two+ years of clinical pharmacy experience preferred.
• Licensure in the applicable state is required.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address all clinical needs.
• Develop medication plans that minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications, dosing, and therapy recommendations.
• Provide guidance on proper drug administration and assess treatment outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise as appropriate.
• Participate in patient assessment and Medication Therapy Services, including dosing, scheduling, routes, and monitoring.
