Mercy is seeking a Clinical Pharmacist to join our East Pharmacy team in Wisconsin as part of the hospital float pool. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You will serve as a drug information resource, participate in patient assessments, and contribute to education, policy reviews, and quality improvement initiatives.

Qualifications

• BS in Pharmacy or PharmD from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).
• PGY-1 Pharmacy Residency (preferred).

Responsabilités

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address clinical needs.
• Develop medication plans that minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.
• Provide guidance on proper drug administration and assess treatment outcomes.
• Serve as a clinical resource for care teams and mentor students/residents as needed.
• Participate in patient assessment and Medication Therapy Services, including dosing, routes, and monitoring.
• Present in-services and contribute to departmental education and quality improvement projects.
