Mercy Hospital Jefferson is seeking a Clinical Pharmacist to join our hospital team, collaborating with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You will serve as a trusted drug-information resource, participate in clinical initiatives, support education efforts, and help deliver high-quality pharmaceutical care in a mission-driven environment committed to Mercy's values.

Qualifications

• Nationally licensed pharmacist (state license)
• PGY1 residency preferred
• Two+ years clinical pharmacy experience preferred

Responsabilités

• Review patient records to ensure appropriate medication therapy.
• Assess conditions to ensure treatment plans address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding meds, dosing, and therapy.
• Provide guidance on proper drug administration and monitor outcomes.
• Serve as a clinical resource for care teams and subspecialty expertise.
• Precept and mentor pharmacy students and residents as needed.
• Present inservices to medical center staff.
