Job summary

Sentara Infusion Services in Charlottesville, VA is seeking a Full-time Clinical Pharmacist for the Home Infusion Pharmacy. The role focuses on comprehensive pharmaceutical care, patient education, and collaboration with healthcare teams to optimize therapy.

You will be responsible for medication-use processes, counseling patients, and contributing to training and development within the infusion services team. This is a full-time position with day shifts in a hospital-like infusion setting.

Qualifications

• Pharmacist license in Virginia is required.
• Bachelor's degree in Pharmacy required; Doctorate preferred.

Responsibilities

• Provide safe, effective, and timely pharmacy services.
• Manage patient drug therapy per clinical standards.
• Ensure appropriateness of provider medication orders.
• Maintain current knowledge of medications and disease states.
• Provide drug information and education to patients and staff.
• Precept pharmacy students as assigned.
