Job summary

UVA Health is seeking a frontline clinical pharmacist to lead a growing specialty home infusion program in Charlottesville. You will manage a dedicated panel of complex patients and collaborate across a multidisciplinary team to optimize therapy outcomes without bedside responsibilities.

The role offers clinical depth, meaningful patient impact, and opportunities to shape workflows, policies, and long-term care models within a fast-scaling, academic health system.

Qualifications

• Education: Bachelor\'s in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.
• Experience: No experience required. PGY-1 residency or equivalent clinical experience preferred.
• Licensure: Virginia State Board of Pharmacy License or within 60 days of hire. American Heart Association (AHA) Health Care Provider BLS certification required.

Responsibilities

• Manage a panel of approximately 50 to 60 specialty infusion patients, supporting them from therapy initiation through stabilization.
• Provide ongoing clinical monitoring, evaluating therapy effectiveness, side effects, and adherence for high-cost, high-complexity medications.
• Conduct proactive patient outreach and education through a remote, telephonic care model, building trusted longitudinal relationships.
• Serve as a central liaison between patients, providers, and pharmacy teams, ensuring seamless coordination of care.
• Collaborate closely with physicians, clinic pharmacists, and internal teams to adjust therapies and resolve complex clinical scenarios.
• Oversee refill coordination and therapy continuity, ensuring safe and timely medication access.
• Participate in weekly process improvement and program development initiatives, contributing to workflow design and policy creation.
• Support operational planning such as high-value medication ordering and forecasting, ensuring alignment with patient needs.

Education
