Job summary

Sentara Health Plans is seeking a Clinical Pharmacist for the Home Infusion Pharmacy in Charlottesville, VA. The role requires managing patient therapies, providing in-depth counseling, and acting as a resource for drug information within a hospital/infusion setting.

The ideal candidate will hold a PharmD (or equivalent) and Virginia Pharmacist License, with BLS certification. This position offers full-time hours, Monday–Friday days, with competitive benefits within Sentara Infusion Services.

Qualifications

• Bachelors Level Degree in Pharmacy (Required).
• Doctorate Level Degree in Pharmacy (Preferred).
• Virginia Pharmacist License (Required).
• Basic Life Support (BLS) (Required).

Responsibilities

• Provide safe, effective, and prompt pharmacy services in home infusion.
• Manage patient drug therapy per clinical standards.
• Provide drug information and education to patients and staff.
• Precept pharmacy students as assigned and participate in training of new employees.
