Job summary

Option Care Health in Chantilly, VA seeks an entry-level Clinical Pharmacist to provide safe pharmacy services in a home care setting. You will coordinate with Nursing and other professionals to educate patients and caregivers about medications.

You will design patient-specific pharmacotherapy plans, monitor outcomes, and ensure compliance with policies and regulatory standards while supporting equipment maintenance and drug information requests.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license in the US.
• Experience or coursework in home care, hospital, or sterile IV admixture preferred.

Responsibilities

• Assess patient suitability for home care and collect relevant information.
• Design and monitor pharmacotherapy plans for home care patients.
• Educate patients, caregivers, and staff on medication use.
• Ensure continuity of pharmaceutical care across settings and maintain proper dispensing practices.
• Maintain equipment and assist with inventory and regulatory compliance.
