UVA Health in Charlottesville seeks a frontline clinical pharmacist to lead a high-growth specialty home infusion program. You will manage a panel of complex patients, monitor therapies, and work with a multidisciplinary team to deliver patient-centered, non-dispensing care.

The role emphasizes clinical decision-making, coordination, and program development, with a focus on sustainability within a 40-hour workweek.

Qualifikationen

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.
• A Virginia State Board of Pharmacy license or license within 60 days of hire.
• AHA Health Care Provider BLS certification required.

Aufgaben

• Manage a panel of approximately 50–60 specialty infusion patients from initiation through stabilization.
• Provide ongoing clinical monitoring of high-cost, high-complexity medications for effectiveness and safety.
• Conduct proactive patient outreach and education via remote telephonic care model.
• Serve as liaison between patients, providers, and pharmacy teams for care coordination.
• Collaborate with physicians and clinic staff to adjust therapies and resolve complex cases.
• Oversee refill coordination and therapy continuity.
• Participate in weekly process improvement and program development initiatives.
