Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services in home care settings. This role coordinates with nursing, educates patients and caregivers, and ensures compliance with policies and regulations.

Graduate of an accredited School of Pharmacy with an active state license is required. Experience with sterile IV admixture or MTM is preferred; strong communication and teamwork are essential for success.

Responsibilities

• Determines suitability of patients for home care and collects info.
• Designs and monitors patient-specific pharmacy care plans.
• Educates patients, caregivers and staff on medication use.
• Dispenses medications following policy and regulatory standards.
• Monitors safety, accuracy and quality assurance in medication therapy.
