Job summary
Option Care Health is seeking a Clinical Pharmacist to provide safe, evidence-based pharmacy services for home infusion patients. This entry-level role coordinates with Nursing and other professionals to monitor progress and educate patients and caregivers on medication use.
The position requires graduation from an accredited School of Pharmacy and an active pharmacy license, plus relevant training or residency in sterile IV admixture or MTM. Compliance with DEA and state regulations is expected.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license.
- Experience/education meeting one of: sterile IV admixture, non-sterile compounding, MTM, residency or ASHP/APPE.
Responsibilities
- Determines suitability of patients for home care.
- Collects and organizes patient information for home care assessment.
- Assesses home care suitability per Option Care policies.
- Develops and monitors patient-specific pharmacotherapy plans.
- Educates patients, caregivers and staff on medication use.
