Job summary

Option Care Health, Inc. is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services to home care patients in accordance with company policies and regulatory standards.

The pharmacist will coordinate patient services with the nursing team, educate patients and caregivers, and support medication-use processes across home care settings to ensure optimal outcomes.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least 1 year of experience in sterile IV admixture procedures, or knowledge of non-sterile compounding, immunizations and MTM, or completion of an ASHP accredited Pharmacy Residency Program, or Advanced Pharmacy Practice Experience, or equivalent experience as approved by VP/SVP.

Responsibilities

• Determines the suitability of individual patients for home care.
• Collects and organizes all patient-specific information needed to determine the suitability of patients for home care.
• Assesses patients' suitability for home care in accordance with Option Care policies.
• Designs, recommends, monitors and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
• Provides medication-use education to Option Care staff, other professionals, home care patients and/or their caregivers.
• Ensures continuity of pharmaceutical care to and from the home and other patient-care settings.
• Presents and dispenses medications following Option Care policies and procedures.
