Job summary

Option Care Health is seeking a Clinical Pharmacist for Ashland, VA. This entry‑level role supports safe home infusion pharmacy services, collaborates with nursing staff, and educates patients and caregivers on medication use in accordance with policy and regulatory standards.

You’ll coordinate patient information, monitor progress toward goals, dispense medications with accuracy, and participate in continuous quality improvement under supervision, with comprehensive benefits and growth

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: ASHP residency, CPD program, or equivalent experience approved by leadership.

Responsibilities

• Determine suitability of patients for home care and collect relevant data.
• Design, monitor and evaluate patient-specific pharmacy care plans.
• Educate patients, caregivers and staff on medication use.
• Ensure accuracy in dispensing and adherence to regulatory standards.
• Coordinate care with Nursing and other healthcare professionals.
