Job summary

Aquent partners with a healthcare organization seeking a Pharmacist to lead medication therapy management across patient populations. This role emphasizes comprehensive MTM, adherence optimization, and safety-driven care in a patient-focused setting.

You will conduct outbound calls, perform medication reviews, and provide detailed consultations while entering data into a secure web-based system. The position supports full-time and part-time availability and requires Florida licensure and US work

Qualifications

• Florida Pharmacist license required.
• 4+ years of clinical MTM experience.
• Strong pharmaceutical vocabulary and clinical knowledge.
• Experience with medication reviews and adherence metrics.
• Proficient in English (verbal and written).
• Able to work with web-based applications and maintain confidentiality.

Responsibilities

• Conduct outbound calls to engage with patients for MTM services.
• Perform comprehensive medication reviews for safety and optimization.
• Provide patient consultations on medication use per guidelines.
• Transcribe prescriptions, diagnoses, and recommendations into the web app.
• Collaborate with a product team on MTM services and solutions.
• Attend regular training to enhance skills and knowledge.
