Aquent is partnering with a leading healthcare organization to transform patient well-being through advanced medication management. The role focuses on MTM, patient engagement, and precise documentation to support safety and efficacy.

You will work with a dedicated team to enhance medication therapy management and patient outcomes, including comprehensive reviews and expert consultations within a flexible work environment.

Qualifications

• Pharmacist license valid in Florida.
• Minimum 4 years clinical MTM experience.
• Strong pharmaceutical vocabulary and clinical knowledge.
• Experience with KPI for adherence and safety.
• Excellent English verbal and written communication.
• Able to work in the US and on a W-2 basis without sponsorship.
• Flexible schedule for full-time and part-time hours.

Responsabilités

• Outbound calls to engage patients for MTM services.
• Conduct comprehensive medication reviews to optimize safety and therapy.
• Provide consultations on statin use in line with guidelines.
• Transcribe patient prescription/diagnosis data and recommendations into a web app.
• Collaborate with a product team advancing MTM services.
• Attend regular trainings to maintain and grow expertise.
