Job summary

Qualifications

• Current KY Pharmacist licensure or eligible for licensure.
• One-year hospital experience or equivalent.
• Hospital pharmacy residency preferred.
• Doctorate Degree or Bachelors.

Responsibilities

• Provide pharmaceutical care activities to ensure safe and effective drug therapy.
• Monitor and intervene in therapeutics to achieve optimum outcomes.
• Consult with physicians and nursing staff regarding patient reactions, errors and complaints.
• Determine and consult on possible drug interactions.
• Assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.
• Monitor, record and communicate using computerized documentation systems.
• Utilize knowledge of age-specific needs of patients in duties.
