Yale NewHaven Health is seeking an entry-level clinical pharmacist to support the Department of Pharmacy in Connecticut. The role focuses on medication safety, patient-centered care, and collaboration with health care teams.

Graduation from ACPE-accredited college of pharmacy with a PharmD or BS is required, with PGY1 residency desirable. Connecticut pharmacist licensure or eligibility is needed; strong communication and teamwork are essential for success in this role.

Qualifications

  • ASHP PGY1 residency desirable, hospital pharmacy experience preferred.
  • Current Connecticut Pharmacist License or eligible for licensure.

Responsibilities

  • Demonstrate CHAMP behaviors and promote patient safety.
  • Review medication orders considering labs and vital signs.
  • Demonstrate competency in pharmacy automation and technology.
  • Adhere to department policies, procedures, protocols, and guidelines.
  • Provide education and training to staff and health care team.
  • Contribute to departmental cost-reduction/revenue generation.

Responsibilities


  • 1.Customer Service Quality

    • 2.1Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs



  • 3.Medication Preparation and Distribution

    • 3.1Demonstrate competency in pharmacy automation and technology as applicable to work area



  • 4.Medication Safety, Regulatory Affairs, and Quality Assurance

    • 4.1Adhere to all departmental policies, procedures, protocols, and guidelines



  • 5.Leadership

    • 5.1Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members.



  • 6.Financial/Volume Results

    • 6.1Contribute to departmental cost-reduction/revenue generation




Qualifications

EDUCATION


Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree


EXPERIENCE


Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.


LICENSURE


Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut


SPECIAL SKILLS


This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
