Job summary

Yale New Haven Health is seeking an entry-level clinical pharmacist to join our hospital-based Pharmacy Department in Connecticut. The role supports CHAMP behaviors, diabetes and medication safety initiatives, and participates in education for staff and students while ensuring high-quality patient care. Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required.
• Current Connecticut Pharmacist License, or eligible for licensure.

Responsibilities

• Demonstrate CHAMP behaviors and promote customer service quality.
• Conduct timely, accurate medication order review considering patient labs and vital signs.
• Demonstrate competency in pharmacy automation and technology for medication preparation and distribution.
• Adhere to departmental policies, procedures, protocols, and guidelines for medication safety and quality assurance.
• Provide education and training to pharmacy staff, students and residents, and health care team members.
• Contribute to departmental cost-reduction and revenue generation.

Qualifications

Special Skills

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
