Job summary

Trinity Health Ann Arbor is seeking a Clinical Pharmacist to provide pharmaceutical care across inpatient settings, including the emergency department and operating rooms, with emphasis on medication therapy management, safety, and cost-effective care.

The role requires Pharm.D. or clinical master's, at least two years of related hospital experience (PGY1/2 preferred), Michigan licensure, BLS/ACLS, and a commitment to CQI and education of staff and residents.

Qualifications

• Thorough knowledge of pharmacotherapy and therapeutics.
• Interdisciplinary communication with health team members.
• Teaching and education of staff, students and residents.

Responsibilities

• Interpret and evaluate medication orders to ensure appropriate therapy.
• Monitor patient regimens for allergies, interactions, contraindications and ADRs.
• Individualize therapy based on renal function, age, weight and labs.
• Collaborate with physicians and nurses to optimize therapy and intervene where needed.
• Participate in rounds, TDM, antimicrobial stewardship and patient education.
• Provide bedside pharmacotherapy support during emergencies and code events.
• Document interventions and participate in QA, drug utilization reviews and pharmacoeconomic analyses.
• Educate staff and other healthcare professionals about cost-effective, safe pharmacotherapy.
