Job summary

Trinity Health Ann Arbor seeks a Clinical Pharmacist for night shifts to provide preparation, distribution and pharmaceutical care to inpatients, ED, and operating rooms. Requires Pharm.D or clinical MS, plus hospital experience and Michigan license; BLS/ACLS as required.

The pharmacist will participate in rounds, therapeutic drug monitoring, patient education, antimicrobial stewardship, and quality improvement initiatives while supporting cost-effective, patient-centered care across age groups.

Qualifications

• Pharm.D or equivalent required to practice hospital pharmacy.
• Minimum of two years related hospital pharmacy experience (clinical residency acceptable).
• Current Michigan pharmacist license or eligibility with BLS/ACLS as required.

Responsibilities

• Interpret and evaluate medication orders for appropriateness of therapy.
• Monitor patient regimens for interactions and contraindications.
• Individualize therapy based on patient factors (renal function, age, weight).
• Interact with physicians and nurses to optimize drug therapy and outcomes.
• Participate in patient rounds, education, and antimicrobial stewardship.
• Document interventions and contribute to CQI and pharmacoeconomic analyses.
• Educate staff and residents on pharmacy care plans and rational drug use.
• Advise on safe, cost-effective medication care for patients.
