Job summary

Trinity Health Ann Arbor is seeking a Clinical Pharmacist for midnights in Emergency Medicine. The inpatient pharmacy operates 24/7 and supports ED, OR and other units with high standards of care.

The pharmacist will provide medication preparation, clinical care and education, working with a multidisciplinary team to ensure safe, effective therapy across age groups. Requires PharmD and Michigan licensure, with experience in hospital settings.

Qualifications

• Doctor of Pharmacy Degree or clinical-oriented Master’s in hospital pharmacy required.
• Minimum of two years related experience; PGY1 required, PGY2 in Emergency Medicine or Critical Care preferred.
• Current Michigan Pharmacist license and BLS/ACLS as required by hospital policy.

Responsibilities

• Interpret and evaluate medication orders for appropriateness of therapy.
• Monitor patient medication regimens for interactions and contraindications.
• Individualize therapy based on patient variables and PK parameters.
• Collaborate with physicians and staff to optimize drug therapy and safety.
• Participate in rounds, antimicrobial stewardship and education for staff and residents.
• Support bedside care during emergencies including trauma, stroke alerts and codes.
• Document interventions and participate in QA monitors and pharmacoeconomic analysis.
• Advise on safe, rational, and cost-effective medication plans.
