Trinity Health Ann Arbor is seeking a Clinical Pharmacist for night shifts in Emergency Medicine. The role focuses on medication therapy management, safe drug use, and collaboration with the emergency team to deliver high-quality patient care. The position requires a Pharm.D.

or clinical hospital pharmacy degree, Michigan licensure, and BLS/ACLS certification as applicable. Experience in hospital pharmacy and emergency settings is preferred.

Qualifications

• Requires Doctor of Pharmacy Degree (i.e., Pharm.D) or clinical oriented Master's Degree in hospital pharmacy from an accredited College of Pharmacy or equivalent.
• Licensed pharmacist with Michigan license or eligible; BLS/ACLS as required by hospital policy.

Responsibilities

• Interprets and evaluates medication orders for appropriate therapy, dosage form and route.
• Monitors patient regimens for interactions, contraindications, allergies and adverse drug reactions.
• Individualizes therapy based on renal function, age, weight and lab values.
• Collaborates with physicians and nurses to optimize therapy and resolves unclear or inappropriate orders.
• Participates in rounds, therapeutic drug monitoring, education and antimicrobial stewardship.
• Provides bedside pharmacotherapy support during emergencies and critical events as needed.
