Job summary

Qualifications

Responsibilities

• Interpret and evaluate medication orders for appropriateness.
• Monitor patient regimens for interactions and contraindications.
• Individualize therapy based on patient variables.
• Manage therapy across care continuum and intervene when needed.
• Participate in rounds, drug monitoring, and patient education.
• Document interventions and support quality improvement.
