Qualifikationen

• Strong verbal and written communication skills.
• Proficiency with computer systems (STAR, Microsoft Office).
• Ability to prepare and deliver presentations to diverse audiences.

Aufgaben

• Provide antimicrobial stewardship/pharmacokinetics, anticoagulation, parenteral nutrition, or other regimens as prescribed by the provider.
• Document clinical interventions on an ongoing basis.
• Round with interdisciplinary teams as able.
• Demonstrate an understanding of distributive responsibilities and computer systems.
• Assist in investigational drug studies when needed.
• Provide on-call clinical pharmacy services as needed.
• Perform medication history review and assist in medication reconciliation.
• Perform medication management per Wellstar policies.
• Develop and deliver education for health care practitioners and patients; serve as a preceptor for students and residents.
• Participate in committees and projects; maintain formulary and ensure regulatory compliance.
