Job summary

Qualifications

• PharmD or BS Pharmacy is acceptable; PharmD preferred.
• Pharmacist licensure (RPH) required at hire.
• Board certification (BCPS, BCCP, etc.) strongly preferred.
• Georgia pharmacist license within 30 days is preferred.

Job Summary:

The Clinical Pharmacy Specialist is responsible for providing comprehensive pharmaceutical care to patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. Individual will be responsible for providing comprehensive medication management through clinical services, patient and provider education, and assisting with ensuring safe medication use on the patients' transition of care (i.e., discharge) from the hospital. Individual may also be responsible for providing recommendations and/or assuming responsibility for patient care activities such as pharmacokinetics, anticoagulation, parenteral nutrition support, or other medication regimens as prescribed by provider.

Core Responsibilities and Essential Functions:

Provide recommendations and/or manage therapy associated with medications, anticoagulation, antimicrobial stewardship/pharmacokinetics, parenteral nutrition or other medication regimens as prescribed by the provider.

• a. Document clinical interventions on an ongoing basis.
• b. Round with interdisciplinary teams.
• c. Clarify medications upon admission and discharge, as needed.
• d. Optimize medication selection to improve medication access post-discharge.
• e. Demonstrate a basic understanding of distributive responsibilities including computer functions.
• f. Assist in investigational drug studies when needed

Educational responsibilities

• a. Counsel patients on their discharge medications/perform MTM services.
• b. Develop and provide educational programs/in-services for health care practitioners and patients as required.
• c. Provide drug information when needed or requested.
• d. Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy.
• e. Serve as a preceptor for the Pharmacy Residency Program (where applicable).
• f. Assist in developing drug protocols.

Participate on assigned Committees

• a. Participate as a member of the Wellstar Pharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary.
• b. Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) Tabulate and present completed MUE data to the P&T committee.
• c. Monitor metrics and develop improvement strategies as necessary.
• d. Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable.
• e. Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements.

Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

• Bachelors Pharmacy or Doctorate Pharmacy-Preferred

Required Minimum License(s) and Certification(s):

All certifications are required upon hire unless otherwise stated.

• RPH - Reg Pharmacist

Additional License(s) and Certification(s):

Board Certification (e.g., BCPS, BCCP, etc.), strongly preferred

Preferred Pharmacist Registration in the state of Georgia within 30 Days Required

Required Minimum Experience:

Minimum 3 years clinical pharmacy experience or PGY1 Pharmacy Residency completion Required and/or

Minimum 2 years clinical pharmacy experience in a specialized area or completion of a PGY2 Residency program Preferred

Experience with nutrition support, anticoagulation management, and pharmacokinetics Preferred
