Aegis Healthcare in Arizona seeks a Clinical Patient Care Navigator to connect with patients, families, and referral partners, expanding our continuum of care across home health, hospice, and related services. You will leverage your clinical background to grow our footprint within West Valley, East Valley, and Tucson territories.

The role requires relationship building, presenting on home health and hospice options, and achieving monthly targets while delivering compassionate, patient-centered

Qualifications

• Hospice clinical background.
• 2+ years of Hospice referral relationships.
• Knowledge of local Senior Care Industry, hospital case management, and skilled nursing.
• Intuitive, solution‑oriented, and driven to achieve goals.
• Detail‑oriented, flexible, and self‑motivated.
• Excellent oral and written communication skills.
• Valid driver’s license (clean record) and reliable transportation.
• Proof of current auto insurance and car registration.
• Valid AZ fingerprint clearance card.
• Current CPR certification and negative TB test.

Responsibilities

• Utilize clinical background to connect with patients, families, and referral partners.
• Strategically expand the business within assigned territory and strengthen partnerships.
• Represent the organization and deliver impactful presentations on Home Health and Hospice care.
• Collaborate with the team to achieve monthly goals and maintain high standards of service and care.
