Thousand Branches Wellness, a start-up-spirited outpatient behavioral health provider in Chino Hills, CA, seeks an experienced Clinical Outreach Liaison to bridge gaps between referral partners and treatment services. The role emphasizes clinically informed outreach, strong partnerships, and coordinated transitions with admissions and clinical teams.

The ideal candidate will hold a Bachelor’s degree, a Master’s in behavioral health preferred, and 1–3+ years in behavioral health.

Qualifications

• Bachelor's degree is required.
• Master's degree in a behavioral health field preferred.
• Active licensure or registration strongly preferred.
• 1–3+ years of experience in behavioral health (clinical, case management, or utilization review experience preferred).
• Strong understanding of behavioral health diagnoses and levels of care (IOP/PHP/residential).
• Established relationships within the behavioral health community.
• Experience with CRM or referral tracking systems.

Through clinically informed community outreach and referral engagement. This role is responsible for the execution of outreach activities with a strong clinical lens, ensuring referral partners are supported in identifying appropriate level-of-care placements. Working within established outreach priorities, the Liaison will serve as a clinical resource in the community, helping bridge gaps between providers and treatment services.

This is a field-based, externally focused role and does not include responsibility for internal program operations, staffing, or clinical service delivery. This position may entail evening and/or weekend hours as needed for community events.
