Job summary

Bon Secours Mercy Health seeks a Nursing leader to guide clinical integration initiatives across Charleston and Florence markets. You will translate data into insights, coach care teams in process improvements, and drive value-based care delivery.

Responsibilities include rounds, outcome reviews, protocol implementation, and stakeholder engagement to improve quality and patient care. RN licensure and experience in population health are required/preferred. Benefits included.

Qualifications

• BSN required.
• MSN or MBA or related Master’s preferred.
• 5+ years in healthcare including ambulatory offices with management/leadership in Population Health or value-based contracts.
• Experience with process improvement models (PDSA, A3) preferred.
• Experience with clinical reporting packages such as Tableau or Milliman preferred.
• Current RN licensure; licensure details not specified; CCNE/ACEN/CNEA accreditation preferred.

Responsibilities

• Completes in-person and virtual rounds to communicate status, drivers of change, and insights to providers and practices.
• Manages review of clinical outcomes, patient panel management, and performance metrics for affiliates and employed physicians in the region.
• Partners with providers to implement evidence-based protocols and improvement initiatives, monitors progress, documents supports, identifies resources, and engages care teams.
• Maintains professional role modeling in practice transformation and educates regional stakeholders on elements, measures, tasks, and tools for practice improvement activities.
• Conducts provider orientation and onboarding/offboarding for panel management and CI initiatives.
• Supports and collaborates with providers and regional leaders to implement processes for value-based participation.
• Collaborates with regional staff to implement network initiatives and support CI activities; conducts chart audits for quality audits under leadership.
• Facilitates remediation tracking and activities with network leadership and quality leadership.
• Engages in relationship management to support ministry outcomes and delivery of care.
