Job summary

Bon Secours in Charleston and Florence areas seeks an experienced Nursing leader to guide clinical integration initiatives and improve value-based care outcomes. You will coach care teams, review metrics, and translate data into actionable improvements across practices and affiliates.

The role requires RN licensure, BSN, and 5+ years in healthcare with population health experience; mastery of PDSA/A3 processes is preferred. Onsite and occasional travel within the Charleston metro will be needed.

Qualifications

• Licensed RN (required) with current licensure in good standing.
• BSN degree required; MSN/MBA or related master’s preferred.
• At least 5 years’ healthcare experience including ambulatory settings and leadership in Population Health or value-based contracts.

Responsibilities

• Conduct in-person and virtual rounds to share status reports and insights with providers and practices.
• Review clinical outcomes, patient panels, and performance metrics for affiliates and physicians.
• Collaborate with providers to implement evidence-based protocols and monitor progress.
• Educate regional stakeholders on concepts of practice transformation and tooling for improvement activities.
• Facilitate onboarding/offboarding of providers into the network related to panel management.
• Support and drive value-based agreement participation through process and quality improvements.
• Coordinate with regional staff to implement network initiatives and process improvements.
• Perform chart audits (not on Epic) to support quality audits and Mercy Health Select distributions.
• Lead remediation tracking and collaborate with leadership for continuous improvement.
• Engage with care teams to support ministry outcomes and care delivery.
