Job summary

Mount Sinai Health System is seeking a Per Diem Clinical Oncology Nurse Navigator to guide patients and families through the cancer care continuum, coordinating follow-up visits and addressing barriers to care.

The ONN will work with interdisciplinary teams to implement evidence-based practices, educate patients, and support decision making while aligning with organizational strategies for high-quality care.

Qualifications

• BSN degree required.
• Master’s degree preferred.
• 3-5 years minimum clinical and administrative nursing experience.
• 2-3 years oncology experience preferred.
• Licensed as a registered nurse in New York State.
• ONS/ONCC Chemo Bio Certification preferred.

Responsibilities

• Delivers patient care by applying the nursing process and organizational standards.
• Leads day-to-day patient care coordination aligned with program objectives.
• Assists patients in accessing cancer care and navigating care systems.
• Assesses barriers to care and collaborates on financial, practical, and social solutions.
• Educates patients and caregivers on treatment plans and roles of the care team.
• Follows up to support adherence and referrals in collaboration with the team.
