Job summary

Qualifications

• Active RN license in the state of WV or multi-state license via eNLC.
• BLS certification obtained within 30 days of hire date.
• Minimum of 3 years nursing experience.

Responsibilities

• Coordinate multidisciplinary care across the care continuum and educate patients.
• Clinically triage new patient referrals to ensure timely access to care.
• Collaborate with the care team to expedite sequencing of patient workup and treatment.
• Communicate and document clinical information between patients and providers.
• Perform targeted symptom assessment and management for high-risk patients.
• Educate patients about diagnosis, treatment side effects, and post-treatment care.
• Assess and document patient needs, goals, and preferences to integrate into care.
• Bridge gaps in care and address barriers to care transitions.
• Document all patient encounters in EPIC.
• Assist with staff education and participate in process improvement initiatives.
• Participate in service line team meetings.
