Responsibilities

• Ambulatory Utilization Management and quality screening for assigned patients.
• Identifies the targeted population and risk stratifies all patients to prioritize needs.
• Coordinates discharge planning with inpatient and outpatient teams.
• Collaborates with physicians, patients, and families to ensure safe transitions of care.
• Designs individualized care plans and coordinates services across the healthcare team.
• Monitors clinical and financial indicators for continuous improvement.
• Coordinates labs and diagnostic testing.
