Concierge Home Care in Orange Park, FL is seeking an experienced Clinical Manager (RN) to lead a dedicated home health team and maintain the highest standards of patient care and regulatory compliance.

The role offers a competitive salary range of $78,000 to $85,000 annually with bonus potential and opportunities for leadership growth within a rapidly expanding organization. The position is primarily in-office with local travel as needed.

Qualifications

• Active Florida RN license required.
• Minimum 1 year RN experience (acute care, med/surg, ICU, ER, or similar).
• At least 1 year of skilled home health experience with OASIS proficiency.
• Strong knowledge of home health regulations and clinical compliance.

Responsibilities

• Supervise and support home health clinical staff.
• Review documentation to ensure Medicare and regulatory compliance.
• Monitor quality of care and patient outcomes.
• Collaborate with physicians and interdisciplinary teams.

Your Role as Clinical Manager (RN)

As a Clinical Manager, you will oversee clinical operations and ensure patients receive high-quality, compliant home health services.

Responsibilities include :

• Supervising and supporting home health clinical staff


• Reviewing documentation to ensure Medicare and regulatory compliance


• Monitoring quality of care and patient outcomes


• Collaborating with physicians and interdisciplinary teams


• Coaching clinicians to support professional development and clinical excellence


• Ensuring agency policies and standards are consistently followed

Schedule

Monday–Friday leadership role (primarily office-based)

Qualifications

• Valid driver’s license, reliable transportation, and auto insurance


• Ability to travel locally as needed to support clinical operations
